Job Description

*Lonestar is seeking to add a RN or LVN Charge Nurse to our HMG team!*

*For immediate consideration please stop by the facility at 2601 Northwest Loop, Stephenville, TX 76401.*
* *6a-6p & 6p-6a Full-time shift available *
* *PRN available*

*Responsibilities:*
* Provide hand on care to the resident based on the resident Plan of Care.
* Administer medications and provide treatment to the resident based on the physician orders.
* Must be able to identify changes in the resident condition and evaluate the resident care needs.
* Strong focus assessment skills as well as documentation skills.
* Must be able to evaluate and participate in the development of the Resident Plan of Care.
* Monitor the resident overall condition and provide care as required while maintaining compliance with the facility policy and procedures.

*Requirements:*
* Registered Nurse or LVN in good standing

*Knowledge / Experience:*
* Prefer at least 1 year of experience

*Licensure / Certifications:*
* Current RN or LVN license in Texas
